20 minutes cold, 20 minutes warm. switch off Hot then cold and repeat a few times, that's what I have heard and also what I do. It's called shocking the nerves in the effected area. Or at least that's what I remember. Works for me.

20 minutes cold, 20 minutes warm. switch off Alternate cold with hot every 20 minutes unless there is swelling, then it's cold only.

my physical therapist says to never put heat on a sore neck or shoulder area the cold reduces inflamation while heat will make it worse.

20 minutes cold, 20 minutes warm. switch off The heat relaxes the muscle and the cold numbs the nerve endings.

Alternate between hot and cold. I know some folks with problems. Their docs have told them to alternate with both. Not too long with ether. The ice keeps inflamation down, the heat can relax.
